Monthly Report Form
The monthly report form can be accessed by clicking on the link to the right. It can be filled out online and e-mailed to us. Here are a couple of things to know that will help you in filling out this form.
- Adobe Reader is required to open and submit this form. To download the latest version of Adobe Reader click here.
- After completing the form, click the Print Form button at the bottom of the page to generate a printout of the report for your records.
- Click the Submit by Email button to submit the form's data by email.
- If you use a desktop email application such as Microsoft Outlook Express or Microsoft Outlook, Eudora or Mail, then a mail message will automatically be generated with the attached data and all you have to do is to click the Send button to send the email to the Food Bank.
- If you use internet email, such as Yahoo or Microsoft Hotmail, you must:
1. Save the form.
2. Open your browser.
3. Navigate to your email.
4. Open a new mail message.
5. Address it to receptionist@cfbeo.org.
6. Attach the saved form.
7. Send the email message.
- If you are unable to submit the form by email, you can:
1. Fax the completed form to (918) 585-2862 to the attention of Receptionist.
2. Mail a copy of the form to: Receptionist, Community Food Bank of Eastern Oklahoma, 1304 N. Kenosha Ave., Tulsa, OK 74106
